Thanks everyone! It's my first time creating an online questionnaire so I am not really expert in how to create it perfectly...it's pretty much test and fix kind of deal for me :P

Please make sure to complete each question until the end of the questionnaire....I rely on everyone's answers because I need to retrieve all the answers and convert them into statistical form....and then doing an indepth analysis on it thank you! :D

* Note: I will only offer gifts to the participants who have reached to the very last page and submitted their username and email addresses to me. If you have not completed the whole survey, please revisit the link and complete it and make sure you enter your username & email address on the very last page. Incomplete questionnaires without confirmation username & email unfortunately means that you are not qualified for a gift.

Instructions:
1) Email me at: eliteresearcher@hotmail.com with the top 3 items you are interested in.
2) Type within the email the item u want most first (in order of priority).
I will review emails and gifts selection according to the date and time of the email. The earlier you email me with your selections, the chances of you getting what you want in priority sequence becomes larger. One gift per person.
3) Location of meetups (Please state which one you are able to meet within the email): Lansdowne Mall (In front of Best Buy outside parking lot), Richmond Center (In front of Tim Hortons outside parking lot), Langara/49th Canada Line skytrain station, Metrotown (In front of Sirens bench seating area), Pacific Center (unknown meeting area at the moment)
4) More preferred day to meet: Saturday / Sunday / weeknight (depending on my schedule)
5) Make sure you enter your username & phone number...this is in case I need to reach you on the day of meetup
6) Once I receive everyone's response, I will begin figuring out what time and dates are for meetup. I am doing full-time study + full time practicum and part time job on the side. Very busy schedule, please be patient while I figure the timing issue.

Note: I am very tired and sleepy, just took snapshots without much photo editing so the photos aren't the greatest under dimmer lighting in my room. Also, will upload more items tomorrow, but if you do end up giving me your selection choice right now...won't be able to switch at a later time. Really busy this semester, so wont be able to change your gift items once you have made your decisions. Good luck in receiving what you want!

Sorry what you mean by you didn't get to enter anything? I apologize but I just checked apparently it does not display the time that people performed the questionnaire. I do have the time of each participant who finished to the very last page and enter their information to me. Those will be the individuals that are qualified to receive a gift. If you have not completed the survey, then you would not have seen the last page where you can input your information for a gift. Please go back to the link and complete the survey ok? :)

Hey sorry, I'm still gonna meet up and all!

I had midterms and assignments over the last 2 weeks and it's been nothing but craziness on my end. I am wondering if there's anyone who would like to meet up over the next 2 weeks during the Olympics?
