Probably a combination of poor diet and steroid/hgh abuse. As much as he is such a nice guy by ALL accounts, he just didn't take proper care of his body. In that PETA video, he is jacked to the gills...

The dead giveaway was the HGH gut he had,that thing was huge,looked like he was pregnant in some movies.

Yup and his huge head... HGH abusers are usually pretty easy to point out physically, as the hormone will enlarge everything including the abdominal muscles, bones, and organs (including the heart)
