No professional lifting experience, but a couple of things:
  • No chocks on wheels. Should chock at least one wheel front and back.
  • Balance of car - front heavy. Front of platform didn't have the lip things that the rear had
  • No one on the corner ropes (whatever you call them) to stop the platform spinning and adding momentum

The car wasn't strapped down at all, it looks like they just had some chunks of wood to chock the front wheels and called it good.

They're called tag lines. And those should only be used to stop the load from swinging when you first lift it, and to stop it from spinning.

The real problem here is the lack of wheel chocks as mentioned, and probably just as importantly, when you are lifting something heavy at one end, a certified rigger will shorten or use shorter slings at the heavy end in order to combat that.

These guys had no idea what they are doing, Crane op is an idiot if he was certified he should have never lifted that load. Chances are he had no experience and didn't give a fuck though.
