I need a record of the shots I have had in the past for a program I am enrolling in for school. Does anyone know who or where I would need to contact to get those records? I know I had shots in school and when I was younger but I can't remember which shots they were etc. and they want proof of it as well.

Your parents, They should have the hardcopy of the information you need.

If you don't, i'd suggest contacting Public Health Authority- they should have full records of your medical history. I believe...

Do you know the number or email of where I can ask i'm having a hard time googling the right place

maybe try your family doctor? they might note have the school immunizations handy but im sure it wouldnt be hard for them to obtain

call Vancouver Coastal Health if you reside in Vancouver

they had my entire immunization records there (from birth to high school ones)

I was born in Vancouver but i've lived in Surrey since I was a baby i'll give them a try though!
