Bought this card from SkinnyPupp back then.
Includes:
- blue case
- CD
- microsd usb adapter
- R4 + 1GB kingston MIJ that came with it.

From what I remember one or two years ago I installed a patched 1.18 custom firmware which ran Chrono Trigger perfectly.
Compatible with NDS and DSLite.

Asking for $25 OBO.
If you rather want it shipped, it'll cost extra.
