Would the medicine still have the same effect if say, you ground the pill up into powder and mixed it in a half cup of water or so?

Depends on the medicine, I'd say. Some pills are designed to dissolve (and thus absorbed by your body) at a certain rate. If you grind it up, you're messing with that property.
