I think you fill in your info and just give the entire paper.
I notice it's also watermarked paper... so they definitely went to lengths to make sure it wouldn't be forged.

Haven't got mine yet. Only ordered one. That means ten more people got to get one for free. Doesn't do me any good if I don't get the only one I requested......... good guys always lose out.
