There's a McDonalds at the Guildford Wal-Mart out in Surrey that does the Big-Xtra, it's weird though because the one 2 minutes from my house doesn't make them...I hope that's changed LOL

In Hong Kong macdonalds, you're lucky if you can get them to add an extra ketchup pack, or switch the mcnugget sauce from sweet and sour (the default) to BBQ 
That goes for all low cost restaurants here. They're not really into changing things at all.
