Yes... sort of.
First, they'd have to get the IP from the forum logs... that would require the forum admin to provide that info, which most would be hesitant to do without a court order or subpoena.
Next, they'd have to contact the poster's ISP to connect that IP to an actual account. The IP would easily tell them who the ISP is, but not the user or his location. Again, most ISPs would not normally do this for anyone who just asked - it would require a request through legal channels.

Most forums would not do this, if for no other reason than opening themselves up to legal liability.

That depends on the monitor used. When I worked at ICBC, all outside internet access required an individual login to the proxy server, and all incoming and outgoing data was monitored and logged. If you said/typed something wrong, they knew about it, and they knew who did it (or at least, who's proxy account it came from... and you were responsible for anything done using your account).
