I heard it can be purchased from chapters? I was browsing but couldn't find it in the menu.

Or you could have one drafted up at a Notary Public/Lawyer for a few hundred bucks if it's nothing too complicated. At least it's tailored to your needs which is how it's supposed to be anyways.
