just one. if you are running OBD2 the second bung is AFTER the cat.

you only need a single o2 sensor for obd-0 if i am not mistaken.

Is the header design for USDM length test pipe or JDM length?

it is generally the midpipe/b-pipe that determines USDM or JDM lengths. the header, with a USDM length midpipe will require a USDM length test pipe/cat. the header with a JDM length midpipe will require a JDM length test pipe/cat.
