Yes, you get offered different items depending on what you answer. Choose "Store Brand" and "Others" to get the most samples.

No, you can do up to 2 maybe 3 times. I just changed my address and name around and it's always worked for me and I get 2 sample packs. What I do is create 2 profiles, fill both up with legit info but change your street info and name info. For example if you live on say Victoria Drive, put "drive" for one profile and "dr" for the other profile. For your name just change the first name.
