this stuff happens all the time...i have heard that the drivers hold their breath for the 4 second run just in case this happens that way they dont breathe in any fumes.

I don't know about any safety measures really helping in this case. Main injuries would've been caused from flying shrapnel, breathing in flames and the shock of the explosion.
