He held the gun gangster style so that he could arc the gun down from above (over the guy's shoulder height), aiming at the head.
Doing this would increase the distance from the gun to smoker's hands, should the smoker decide to try to make a move to grab the gun.
I don't think he was just doing it to be gangster.
