If you seen him sneaking drinks, you don't need an excuse just bring it up... you don't even need to bring up the liver problem just say hey if you're hiding your drinking there's a problem bro are you gonna do something about it or what's your long term plan here you know it's bad for our health as we get older... something like that and just take it form there? He might even tell you about the liver issues on his own if they're true.
Next time you see him, do the alcoholic a favor, and bring a bottle.

When i was like 11 years old, my mom would deport me back to the motherland every summer, i stayed on a backyard farm with my moms friends.
The dad, liked to drink.
The mother, did not like that the dad liked to drink.
I had two whole months of doing nothing, a couple times a week i would run down to the corner store with like 20c that i would find on the ground to buy bubble gum or whatever.
Sometimes the dad would see me leaving, so he would "take out the trash" in the exact same time.
He would catch me just outside their complex door, or such and it would go like this: "hey HEY donk, wait up! i have an extra 60c for you, if you just bring me back a bottle, but be QUICK!, i only have a few minutes before the wife starts asking questions!"
So anyways i would run down with my extra 60c, super excited, get to the corner store, pick up a 2L bottle of beer, and some extra gum or candy for myself, then run back to the front of the gate where the dad was waiting for me. (Yes they will sell to beer kids in ukraine, they dont care, same goes for cigarette)
By the time i got to the gate, i was greeted with the biggest smile from the dad, eyes glossy, soul glowing, the best part was watching him CHUG the shit out of this 2L bottle within seconds, i looked up at him, like a god, chugging that shit as if it was his last meal.
Every. Time.
I wish that man was your father just to see how you would have turned out lol
Hang out with him for a full day. Sneak your friend 30g lactulose with each meal, ensure he's had 3 bowel movements, then try talking to him during after dinner dessert about drinking too much.
I'd say something man, depending on how close you are to this guy.
If you're close friends it behooves you to talk to him, even from the angle of you being worried about him, and caring for his health.
You might be a bit of a busybody Peanut, but you sound like a good guy.
It's corny and been said many times, but men should be more comfortable talking to their friends about how they feel.

end of the day, the change has to come from himself. i've seen patients drink to the point where they get seizures and even that isn't enough to stop them. many of them know they're on the path to death's door and STILL won't stop. I had one pt who was my age and she eventually passed away in ICU.

Does your friend have children? If he does, appeal to his desire to be a good father and tell him to stop drinking so that his children don't see him die young.
If that doesn't work, there's definitely a stigma against alcohol these days, particularly among middle-aged men. Send him a few YouTube clips about middle-aged men cutting back alcohol consumption, self-improvement, etc. and let the algorithm do its work.

Thanks for the input. I feel you guys are right, I always seem to get too involved with peoples lives. He hasn't told me about his condition so he's either ashamed to tell me or he doesn't want to tell me.
I'm going to see him on Wednesday night actually, my other buddy has a check engine light on his 2004 X3 so it's just an excuse to all hang out. I don't know sh*t about cars, but I like to hang around. I can't imaging he'll bring it up then, but the most i'll do is just ask him how things are going and leave it at that.
Even if he tells me, I still wouldn't know what to tell the guy. I think alcoholics know they shouldn't be drinking.
He has two young boys, four and one. So there is definitely some incentive to stop drinking, but like some of you said, gambling and alcohol is a tough vice to get rid of.
Too bad it isn't about flipping cars to lose money, I'm really good at that.

A few years back I got fired from my job (I, as a VP, very undiplomatically and publicly called bullshit about some culture stuff that the head of HR really cared about and it turned out she was dating the CEO) - so two of the guys in my group chat go on Cameo and hired William Hung to record a video to console me.
