#2 isn't really that big of an issue.
The minute you open your gas cap, there is a chance gas vapors can escape. I know for sure when you open the gas cap to the tank on a motorbike.

Car gas tanks have a flap so vapors don't escape. When you insert the nozzle it opens it up.

Lol, sometime it's like when people go to a restaurant and tip enough to make the total an even number; at the end of the day it doesn't make a difference, if you're paying by Visa you're probably still going to end up with an un-even total on your statement.

I usually fill up when im dead empty and when the pump stops there's usually a couple liters extra for me that I could put in after it clicks.

Personally I'd only care about #1, but that's enough for me not to do it
I admit I top up when I get gas in the States. I think I usually put in at least another 5L after the first click, which is like saving another buck.

#1 and #4 is not applicable for me though, because it's not like I keep pumping it with the nozzle inserted. I pull it out and slowly top it up.

I've probably done this a hundred times in my Yaris. Haven't had any problems yet.
