3 weeks paid vacation, but any days working trade shows and out of town conferences are banked, so I ended up with roughly 4 weeks paid vacation in 2012. I already have a week's worth banked for this year. Been with company 4 years. Not sure how many sick days and personal days I get as I never use them. Photographic industry.

First year - 2 weeks
2-6 years- 3 weeks
7-11 years - 4 weeks
more than 12 years - 5 weeks
Also get 80Hrs of sick time (10days), all 100% company paid. Can work OT and bank up to 173hrs for extra vacation time

banking industry
- after 3 mos probation - 3 weeks based on guaranteed hours/week of work.
if my hours/week is 20 hours, then i get 20x3 hours of vacation.
- not sure when the next tier is. but max is 5 weeks
- vacations have to be reserved at the beginning of the year based on seniority
- also no limit on sick days (paid) but it has to be reasonable.

2 weeks paid + up to 2 weeks unpaid for a total of 4 weeks after 90 day probation
I think the next tier is at 5 years with 3 weeks paid + up to 2 weeks unpaid
and I think it tops out @ 7 weeks paid + 2 weeks unpaid (but i think thats like 25 years+)
vacation is granted based on availability and seniority, but xmas is blacked out and cannot be booked off...
unlimited non-paid sick days
but the rules can be ignored at the discretion of the manager. (so just kiss ass)
I am going back to asia for 5 weeks in march, Ive told them since aug of last year...
since I have only been with the company for 1 year, the way my supervisor did for me was 4 sick days + 2 weeks unpaid vacation + 2 weeks paid vacation and a 4 hour shift on the day I am back since my plane arrives @ like 9am

been with my company for 4 years before I quit on happy terms (I'm still working for them over the internet, putting in around 10 hours/week)
Being a small company with only 6 front office staff, holidays were lenient given that you have everything done and the boss clears you.
Last year I got 4 weeks of paid holiday, and another 2 without pay. The paid holiday though I was expected to put in about 1 hour/day given that I had internet connection to tend to the office computer network and such.

vacation
1 to 7 years of service: 105 hrs (three weeks) of vacation per year
8 to 15 years: 140 hrs (four weeks)
16 to 23 years: 175 hrs (five weeks)
24 years +: 210 hrs (six weeks)
*note* - each work week is only 35 hrs long.
from 8 years of service onwards, you have to take at least 105 hrs (three weeks) of vacation per year and can bank the rest to a maximum of 140 hrs (four weeks) of banked vacation.
in addition to the above, at 11 years of service and every five years thereafter (@16, @21, @26, @31, @36, @41, and @46 years of service), you get an additional 35 hrs (1 week) of supplementary vacation leave. supplementary vacation can only be banked every five years.
net result thus far is that at 46 years of service, you get six weeks of vacation per year. Plus, you get 8 weeks of supplementary vacation that you have to use by your 51st year of service.
if you are entitled to 175 hrs of vacation a year, you can bank 35 hrs (1 week) of vacation per year into the early retirement bank. if you are entitled to 210 hrs of vacation, you can bank 70 hrs (2 weeks) of vacation per year into the early retirement bank. you can only use this particular banked vacation immediately prior to retirement.
sick leave
first 6 days of each absence due to illness are paid at 100%. after the 6 days are used, then each sick day thereafter (medium term sick leave - for the next 16 continuous weeks) is paid out at 75%.
for every four months of a year that a sick day is not used, you receive 7 hrs (1 day) gratuity leave (days off) at the end of that four months, up to a maximum of 21 hrs (3 days) per year. gratuity leave can be rolled over from year to year to a maximum of 120 days.
this is as an employee for a local municipal government.

3 weeks vacation.
unlimited 100% paid sick days.
Can work OT whenever and bank days.
option for compressed work days to also get another day off every 2 weeks.
and I think 4 weeks vacay after 10 years or something.
boxing day and newyears count as stat holidays. and christmas and newyears eve are full day paid and only have to work 4 hours.
surprised no one mentioned sabbatical? 4 weeks sabbatical after 5 years.
