I'm about to use 5DPackages. But I'm not sure where to put the PMB#. Do I put it within my address? Or put it beside my Name? It's all YMMV. I've been taxed on $200. I've been let go with $500+ worth of goods often. Just answer the questions and be precise with your declarations. The less vague you are the less time you need to spend at the booth. |

I'm surprised that they would send you a NAFTA form. You'd essentially have to fill in one of these for every item you bring over as the origin/manufacture location varies for each item. Are you sure it's not the USPS form for delivery through an agent? I'm pretty sure they make everyone who delivers to any mail receiving place fill in one of these when they sign up.
