I work alongside NPs in my practice settings and they fill an important (and massive) gap in our system. I obviously think that having a competent NP is far better than not having a family doctor at all.

However there are some implications that are yet to be understood. One example is that anecdotally and very generally speaking, many of my colleagues have the sense that some NPs have a lower threshold to refer (i.e. to specialists) and/or investigate (e.g. blood tests, CT scans, MRIs) that a family physician may have worked up/managed without. While patients may like that, this is not always something that improves outcomes (and in some case, can lead to bad outcomes, e.g. with inappropriate investigations) and there is a health system cost to that. There are other differences I won't go into as well, e.g. remuneration model, liability, etc.

Again, big asterisk as there are both excellent and less excellent family docs and NPs on both sides.
Good to hear a take on it that’s far more knowledgeable about it than I am. Not to take anything away from GPs and GPs training/knowledge level although it may have read like that when I look at my wording. More about how the general public really often only uses a GP for the simple things and wastes a lot of time in other areas as a result, including the GP
To try and quantify it, it’s like people go to GPs for things that are 1-5 on the scale of importance, then visit an ER for things that are 3-10, where the system would be so much more cost and time effective if people were going to NPs for the 1-4, GPs from 4-8 and ER for 8-10. Of course people are going to the ER for the 1s and going to GPs for the 10s but I’m just generalizing how the majority of people work in this system and how it creates so much waste.

I also like how for level 1-2 things, pharmacists (doctor of pharmacy now) can prescribe things. You'd think this would also alleviate some of the time of GP's.

And non-urgent care centres, that also has done some positives to alleviate Emergency Depts.

Some improvements from a front end user pt of view.

On the topic of healthcare, you guys should see how many repeat meth lovers the ER departments see PER DAY. They really gotta implement a 3 strike rule.
Smoke meth once? Revived.
Smoke meth twice? Revived with strict warning.
Smoke meth thrice? Revived and banned from public healthcare. You want help? Pay for it out of your own pocket.

Theres no reason someone that actually needs emergency care, needs to wait 2-8hrs, for help, because someone has overdosed for the 28th time.

A $10 Starbucks drink 365 days is only $3650 a year, which is enough to pay most condo property tax + insurance

It is an unpopular and uncomfortable discussion, and all the bleeding heart people in society are gonna condemn the hell out of any resemblance of this discussion. But the fact of the matter is, hospitals already triage people, and doctors have already had been put into that unenviable position before during COVID in choosing who gets a chance to live, vs who's gonna be left for dead.

Also, any politician who dared to bring up such a topic is pretty much committing a self-inflicted political suicide, so you can bet absolutely nobody is gonna try and suggest something like this.

At the same time, there are social healthcare programs like this that seems to be very effective in dramatically reducing repeated ER visits from "frequent flyers" while simultaneously helping them, as well as bring in massive financial savings:

https://www.cbc.ca/news/canada/toron...ital-1.7650086

So while the hardline Conservative side of me agree with the 3-strikes and you're out model, the progressive side of me also thinks social-medical programs like these could be a much better answer to the ER overuse by a selected group of frequent users.

Back to rentals. Property management is a hard or as easy as you want to make it. Had a condo in Maple ridge that I never saw. Never even got the keys. A bunch of family members also bought in the same building. Property manager who charged 5% handled everything. I dealt with maybe 3 or 4 emails a year plus some stuff during tax time. I also own a house that I manage. The tenant is a single mom with 2 kids and 3 dogs. She is not going anywhere. Rent gets E-transferred every month. I do next to no work.

People say a house is a home. it's also the biggest financial investment most people are going to make. So make it count. Think about how else you make an extra $1000 plus a month without a rental unit. Drive Uber, do Skip the dishes, start an OF page?

The grass is on them. She has 3 dogs and a large above ground pool so the grass is probably fckd. As for repairs we have random semi reliable handymen that can do most of the work. Last year the dish washer died. I bought a new one at lowes and had them deliver and install it.

how the fuck do you spend it all
Not hard to spend that much to be honest. That 169K is gross and total comp - she's a nurse so she's putting in 9-10% pre tax into a defined benefit pension. It also includes the cost of other benefits that the employer is paying, including the employer's share of the pension, which is likely 9-10%, if not more.

Hair is like 500-1000 every 2 months. Sephora is easily $5K per year. Shopping for clothes per week can be like $500-1000 for a few simple pieces at Aritizia. Handbags from designers other than Coach are easily 1K-4K a pop. Pilates memberships. Spa treatments. Botox and other injections every few months. It all adds up.

A trip involving a plane overseas is easily 10K between 2 people if you fly premium economy, stay in 4 star and up hotels, eat at fancy restaurants, shop, spa, etc.

If you're doing multiple trips per year involving long haul flights, 10K is nothing for those types of trips. Probably looking at least $5K per person a long haul trip.

She was carrying a $2500 Goyard tote at the noob meet. Like Gerbs says, you two need to get to a honest space about your respective finances. $40k consumer debt is worrisome. My wife's in the financial planning world, and the number of high income earners living paycheck to paycheck is frightening. Like Dukes said a few pages back, lifestyle creep is a real thing.

It's an addiction fueled by the high income. A neverending cycle.

At first, it's a treat for a job well done, then it's a reward for some milestone, then it's 'the weekend' so why not? And then it's a friend's birthday so let's 'live it up'. You see how this spirals so quickly?

It's a sign of life without meaning and purpose. Just the high of mindless product acquisition but nothing deeper. Re-evaluate your life and your place in the world and feed that inner spirit and be productive out there.

On the path to rehabilitation, don't try to obsess over how far you need to go, take joy in how far you've come already.
